We look forward to working with you with you during our 2020-2021 Performance Recreational Season. Be sure to arrive with these items already completed or turned in by your first practice.

• Athlete Information and Liability Waiver 

• Copy of Birth Certificate 

• Financial Agreement 

• Payment Authorization Form 

• Registration Fee and Uniform Payment (if not pre registered online)
